Bear Creek Services is actively hiring a Program Coordinator in Rochester, MN. Candidates who are eager to identify career goals and have a desire to build a career path within the organization are strongly desired.

In this role, you will supervise the daily operation and all related activities of the group home. You will develop and utilize systems to ensure individuals progress on an individualized program while coaching and mentoring.

Job Duties:

  • Support Direct Support Professionals, review staff assignments, coordinate activities, and resolve work-related issues
  • Instruct, mentor, and assist staff in coaching individuals in their daily living activities
  • Participate in the recruitment and hiring processes and disciplinary and termination processes
  • Design, implement and monitor programs to meet individual’s person-centered planning goals
  • Participate in meetings with service providers, interdisciplinary teams, professionals, families, and other agencies to discuss individuals’ needs

 

Job Requirements:

  • Valid driver’s license, required
  • 2+ years of developmental disability caretaking experience, required
  • 1+ year supervisory leadership experience, required

 

Education Requirements – candidates must have one of the following:

  • Bachelor’s degree in a field related to human services plus one year of full-time direct care experience
  • Associates degree in a field related to human services plus two years of full-time direct care experience
  • Diploma or Certification in a field related to human services from an accredited post-secondary institution plus three years of full-time direct care work
  • HS Diploma/GED plus four years of full-time direct care experience
