Job Description
Service: Registered Nurse - Directorate of Medical Services Place of Performance: The work to be performed under this contract will be at Navy Medical Center San Diego (NMCSD)Recognized Holidays: Contract HCWs will be required to work on federally recognized holidays. * New Year's Day, January 1st * Martin Luther King's Birthday, 3rd Monday in January * President's Day, 3rd Monday in February * Memorial Day, Last Monday in May * Independence Day, July 4th * Labor Day, 1st Monday in September * Columbus Day, 2nd Monday in October * Veterans Day, November 11th * Thanksgiving Day, 4th Thursday in November * Christmas Day, December 25thNOTE 1. Any holidays that are declared by Presidential Executive Order shall be observed in the same manner as the holidays listed above. If the area in which a contract employee is scheduled to work is closed for any reason and the employee is not required to report in, payment will not be made for those hours. Hours of Performance: The performance hours of the MTF are as follows: Registered Nurse (Medical Home Port, Primary Care clinics for the Director of Medical Services) Work Schedule: - Services shall be provided Monday through Friday, between the hours of 0600-1800 for an 8.5, 9, or 10.5 hour shift which include an uncompensated 30 minute or one-hour meal break depending on length of shift. An alternative work schedule is available to include working (4) 10 hour shifts for a work week but not to exceed 80 hours in a two-week period. The specific duty hours for each HCW will be scheduled 45 days in advance by the Government. In no instance will the HCW be required to provide services in excess of 80 hours per two-week period. Closures: During a planned closure of the facility due to training, holiday or unplanned closure due to unusual and compelling circumstances (e.g., natural disasters, military emergencies, severe weather), the Contractor willbe compensated for the unplanned closure. Mission Essential: The following labor categories are designated as mission essential: LVN (Family Practice, Pediatrics, Internal Medicine) Legal Authorization to Work in the United States: HCWs performing under this contract must be a U.S. Citizen as defined by the Department of Homeland Security, U. S. Citizenship, and Immigration Services, either a noncitizen national of the United State, lawful permanent resident of the United States, or alien authorized to work in the United States.
Labor Category: Registered Nurse - Director of Medical Services Minimum Qualifications: *Degree: Associate's Degree in Nursing. *Education: Graduated from an accredited registered nurse program. Possess and maintain a current unrestricted license to practice as a Registered Nurse in any 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S Virgin Islands. This position is subject to credentialing. *Certifications: American Heart Association Basic Life Support (BLS) or American Red Cross Basic Life Support (BLS) for Healthcare Providers or American Heart Association or American Red Cross Healthcare Provider Course. *Experience: Minimum three years of clinical registered nurse experience within the last five years. *Licensure: Possess and maintain a current unrestricted license to practice as a Registered Nurse in any 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S Virgin Islands. GENERAL DUTIES: The HCW shall provide services within the scope of their competencies and the applicable duties provided in the basic contract. Additionally, the HCWs shall perform the following: *Maintain a level of productivity comparable with that of other individuals performing similar services. *Participate in peer review and performance improvement activities. *Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection. *Function with an awareness and application of safety procedures. *Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, director, or other designated person regarding problems that the HCW is unable to manage. *Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner. *Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting. *Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF. *Maintain an awareness of responsibility and accountability for own professional practice. *Participate in continuing education to meet own professional growth. *Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF: disaster training,
infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training. *Actively participate in the command's Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. *Attend and participate in various meetings as directed. *Perform timely, accurate, and concise documentation of patient care. *Operate and manipulate automated systems such as MHS Genesis and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications. *Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions. SPECIFIC DUTIES: Provision of Patient Care *Provide comprehensive nursing care for all ages and families in their Medical Home Port Team utilizing evidenced-based clinical practice guidelines and engages patients actively in their health care. *Demonstrate clinical expertise in the execution of the nursing process and clinically directs staff in its application. *May administer routine intravenous fluids and/or medications. *Monitor and/or administer diagnostic and therapeutic agents. *Initiate and maintain medical history on patients, recording observations and stated symptoms for use by the Medical Home Port Team. Observe and record pertinent vital signs, determine, and order laboratory procedures on patients that may be required by the provider to diagnose and treat the patient, per command and department policies. *Perform triage through the screening of patient problems. *Take appropriate action such as practitioner referral and evaluation, health care advice, initiation of medical protocols or referral based on medical staff protocols. Medical Home Port (MHP) Team *Proactively collaborate and integrate with members of their team, other departments, and directorates to achieve optimal care for the patient in the HOME PORT Model. *Participate in continuous process improvement to improve access to care, quality of care delivered, experience of care and to demand manage the utilization of health services by enrollees. *Actively participate in MHP Team huddles *Proactively assist team's identification of diagnostic procedures required during schedule scrub. *Assist with identification of patient education needs and reviews plan with team members for coordination of educational needs.
Assist providers with physical examinations, treatments, and diagnostic procedures. *May assist provider with completion of reporting and required documentations regarding communicable disease and sexually transmitted diseases to the proper authorities. *Maintain effective communication with staff, patients, families, and between other departments. - Act as a patient advocate by facilitating communication for the patient with other departments and their staff. Communication avenues may include asynchronous messaging to improve communication, telephone consultations and other non-traditional means. *Engage other MHP Team members to assist in improving documentation of the episode of care using MHP Clinic-approved templates - Reason for today's visit, History of Present Illness, Past Medical and Surgical History, Social History, and Medications with Medication Reconciliation. *Train hospital corpsman (HM)/Licensed Practical Nurse (LPN)/Certified Medical Assistant (CMA)/Medical Support Assistant (MSA) in meaningful and appropriate documentation regarding the patient's visit. Clinical Direction *Coordinate the health care needs of the patient utilizing the full scope of the Medical Home Port team, external specialties and call systems, as applicable. *Identify new clinical and management requirements and takes effective action to ensure necessary changes and adaptations are implemented. *Participate in periodic in-service programs for the continuing education of staff. *Plan and implement performance improvement activities based on the Process Improvement plan for the command. Population Health *Proactively coordinate the screening of their enrolled patients to identify patients needing preventative services and education based on their specific health care needs including, but not limited to: health and wellness topics, medical conditions and medical procedures. (Examples include mammograms, A1C, etc.). *Ensure review of emergency treatment records (ETR) and follow up with patient, per command and department policies. *Track Healthcare Effectiveness Data and Information Set (HEDIS) measures, lab/radiology results and notify patients per command/department policies. Performance Evaluation and Improvement *Maintain current knowledge of new developments in clinical specialty and ambulatory nursing. *Demonstrate awareness of and adherence to safety and legal requirements at the command and department levels with emphasis on those pertinent to the clinic. Collaborate with staff and support activities to maintain a safe and orderly department environment.
