  • Under general direction of the PACE leadership and the Assistant Medical Director of PACE and Older Adult Services, develops, implements, and manages the quality improvement program for the PACE program in accord with CMS and DHCS requirements. Completes the annual review of the quality program and updates of quality improvement plan as needed. Assist with implementation of quality indicators, data management, education, and training of staff around quality improvement to include preparation of routine and special reports, and coordination of improvement efforts arising from monitoring activities. Investigates patient incidents, identifies root causes, and collaborates with the clinical staff and implementing counter measures. Monitors the patient care and care process and identifies gaps in care and alerts PACE Leadership, the Assistant Medical Director of PACE and Older Adult Services, and FHCSD executive leadership, as necessary. The employee may be requested to perform Level of Care assessments and submit the applications for PACE to the California Department of Healthcare Services (DHCS) attesting to the state that the senior is appropriate for PACE.
  • Job Roles
    • Coordinate quality assurance/improvements activities based on the CMS and DHCS requirements.
    • In collaboration with the PACE leadership, maintains the PACE Quality Plan and annual QAPI program effectiveness review,
    • Coordinates the Participant Advisory Council, participant/caregiver satisfaction survey, and the CMS required HOS-M annual survey.
    • Leads quality improvement initiatives along with key members of the PACE interdisciplinary team
    • Coordinate identification of program-wide specific quality indicators.
    • Conduct in-service training for PACE staff.
    • Facilitate quality assurance-related communications to the interdisciplinary team.
    • Perform data collection, analysis, and reporting as required.
    • Assist PACE management in preparation for certification surveys and involve staff at all levels in survey compliance issues.
    • In collaboration with PACE management team, coordinate and facilitate all State and federal rules on compliance updates and implementation related to the overall integrity of the quality assurance program.
    • Perform contractor onsite quality reviews.
    • Regularly enters quality data into the CMS portal (HPMS) as required.
    • As time permits, the employee may be asked to assist in completing Level of Care Assessments for prospects for the PACE program. This would entail completing a home evaluation and submitting the application for enrollment to DHCS.
    • Perform other duties as assigned.

    Education/Certifications/Licenses/Registrations

    • California State Board RN License required.
    • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training.
    • Graduation from a recognized RN/BSN program.
    • Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No more than 2 violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years.
    • Professional certification is preferred.

    Experience/Specialized skills (including Language)

    • 2 years of experience in compliance or quality assurance.
    • Demonstrated experience in performing tasks related to quality assurance processes, data collection, analysis, and/or systems analysis.
    • Strong analytical, organizational, and interpersonal communication skills (written, verbal and presentation).
    • Demonstrated experience preparing comprehensive, accurate, and detailed analyses.
    • Technical proficiency with the Microsoft Office suite of applications.
    • Experience identifying and presenting unfavorable or controversial information with tact and skill.
    • Experience working with accreditation or regulatory standards highly desirable

$40.00 - $49.95

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
