We have over 30 outpatient behavioral health locations nestled throughout our communities located in Kern County, Los Angeles County, Orange County, and San Diego County.
Pathways is seeking an innovative and purpose driven individual to join our team as a Family Support Partner.

Cornerstone is a school-based, full-service partnership (FSP) program serving Medi-cal eligible and unfunded youth (*ages 5-21) and their families. Cornerstone is currently partnered with 19 school sites within San Diego Unified School District and serves the Mid-City area of San Diego. Services include individual therapy, family therapy, case management, crisis intervention, psychiatric evaluation and medication management.

Summary:
The Family Support Partner is responsible for providing direct services to clients and overseeing event development. The lead also assists with day to day operations under the Program Director, providing support with ) Families.
Pay:$19-$22hr

Essential Duties/Responsibilities:
  • Provide client and family support
  • Transport clients to accommodate community relationships. Use of personal car or vehicle is required
  • Mentor families in communication skills, anger management, problem solving, stress management, appropriate discipline and other life skills
  • Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients
  • Assist families in contacting and utilizing community resources
  • Participate in development of family plans
  • Assist daily operations per site needs
  • Identify outreach opportunities to create new cohorts and build relationships with providers in the community
  • Other duties as assigned or necessary to support the program and/or the company

  • Create an organizational culture that respects and celebrates the diversity of our consumers

  • Research and utilize our industry's best practices and analyze our own services to ensure the best possible outcomes

Education/Licensing Requirements:
Education and/or Experience
Bachelor's degree in Human Services or related field from an accredited college or experience receiving mental health services or are a family member of someone receiving mental health services

Certificates, Licenses, Registrations
Proof of: valid California driver's license and auto insurance, as well as proof of education are required

Knowledge, Skills and Experience:
  • Excellent prioritization and organization skills, strong interpersonal skills and excellent written and verbal communication skills
  • Intermediate level PC skills required
  • Ability to work flexible schedule to meet the needs to the Program
  • Ability to effectively prioritize and manage time
Physical Requirements:
  • Office Environment (not remote position). Requires extensive sitting with periodic standing and walking
  • May be required to lift up to 20 pounds
  • Requires significant use of computer, tablet, phone and/or general office equipment
  • Needs adequate visual acuity, ability to grasp and handle objects
PAY$18-$20/HR
