We are looking for a Licensed Social Worker - LSW - Part-Time.

** $29 to $31 per hour based on experience and skills **

** 10-minute commute from Cincinnati **

Licensed Social Worker - LSW Job Summary:

• Assist with necessary documentation for the resident’s admission to the facility. To include, but not limited to the Social Service Evaluation, Discharge plan, Initial admission Progress note and Geriatric Depression Scale.
• Assist with interviewing applicants and families to determine suitability of residency.
• Interview and access Resident with input of family to determine resident need such as psycho-social strengths/ weaknesses and assist with the adjustment of the resident through planned interventions.
• Assist to update the resident files/charts with the Discharge Plan, Progress Notes, Plan of Care and Assessment.
• Participate in the Interdisciplinary Care Conference Meetings to assess and review Residents’ progress.
• Assist resident and/or family members with issues concerning but not limited to emotional and financial needs.
• Assist in communication with families and notify them of new and apparent needs or any changes of the level of care.
• Conduct duties of the Admission Coordinator when coordinator is not available.

Licensed Social Worker - LSW Qualifications:

• Valid KY Social Worker license required.
• Masters in Social Work required.
• Skilled Nursing Facility (SNF) experience preferred.
• Knowledge of MDS 3.0
• Knowledge of Point Click Care or other Electronic Medical Records (EMRs).
• Read, write, and speak English fluently.
