Social Worker - LSW - Nursing Home

We are seeking to hire a Licensed Social Worker.

Social Worker - LSW - Long-Term Care Job Summary:

• Schedules interdisciplinary team meetings to plan care and resolve concerns.
• Ensures resident adjustment to rehabilitation or long-term care by performing psychological screenings, initial plan of care, introduction to services and procedures.
• Completes all necessary paperwork including MDS.
• Reviews goals with resident and family member/responsible party if they are unable to attend care conferences.
• Facilitates communication with care providers as necessary.
• Documents all communication in medical record.
• Collaborates with families, Business Office to assist with applications and qualification for financial assistance.
• Demonstrates understanding and provides guidance and counseling to residents and their families on diverse issues ranging from grief and loss to acceptance and coping with changes in living arrangements.
• Coordinates discharge services - planning and acquisition of appropriate follow up care.

Social Worker - LSW - Long-Term Care Qualifications:

• Must be a licensed Social Worker in Massachusetts, but let's talk if you're sitting for the exam.
• MSW or LMSW required.
• Experience in a Long-Term Care and good discharge experience preferred.
