Position Title: Community Action Team (CAT) Team Nurse
Location: Gainesville, FL, Lake City, FL
Salary: LPN: $23.69.02 | Hr. | RN: $35.02 | Hr. | Non-Exempt

• Monitor all aspects of the medication regime for CAT clients.
• Provide specialized services, i.e., medical assistance, substance abuse education, health and wellness education.
• Assist Psychiatrist with medication monitoring for clients.
• Monitor the health and dental needs of the clients.
• Document work in the Electronic Medical Record.
• Act as a participating member of the interdisciplinary CAT.
• Consult with other medical personnel to ensure proper consumer follow through and growth.

Who are we looking for?

• Clear/Active license to practice as an LPN or RN in the State of Florida.
• Two years’ experience preferably with patients who have substance use and/or co-occurring disorders.
• Current Florida Driver’s License with a clean driving record while meeting company insurance requirements.

$5,0000 sign-on bonus for all RN’s
