What does this role entail?
- Serves as operational program lead for implementing and delivering our community program models through various funding sources
- Lead efforts expand and enhance service delivery for chronic disease prevention and management programming, health education and literacy, and other supports/programs related to social determinants of health; ensure high-quality program delivery that maintains fidelity to curriculum and results in measurable health status improvement of participants
- Effectively manage day-to-day Community Health and Well-Being operations, including monitoring of departmental expenses, staffing, scheduling, facility spaces and interdepartmental coordination to ensure an optimal participant experience
- Manage and coordinate individual supervision, group supervision, staff, program and community meetings; Attend countywide contract-related and collaborative meetings
- Lead and manage data and evaluation efforts, including accurate data entry, data analysis, continuous quality improvement and grant reporting requirements
- Lead all partnership development and cultivation; identify potential partners, engage in collaborative partnership planning and maintenance; Partnerships may focus on leveraging programs, services, resources, referrals and/ or recruitment of participants
- Handle on-site participant crisis situations, over the phone crisis calls, and referrals
- Support planning for growth and long-term funding, staffing, partnership, and resource sustainability
What do I need to bring to the table?
- Bachelor’s degree in Exercise Science, Public Health, or related field or equivalent experience
- Strong computer knowledge and skills
- Experience implementing evidence-based program models, including targeting specific or underserved populations preferred
- Experience managing referral relationships with health care providers preferred
- Experience implementing quality assurance and data collection protocols preferred
- Blingual preferred
- Experience with supervision preferred
- This position requires sitting and standing for extended periods of time
- Visual and auditory ability to respond to critical incidents and the physical ability to act swiftly in an emergency situation
- Licensing, state law and our funders require that applicants of critical positions be fingerprinted, prior to reporting to work, and include subsequent arrest notifications
- CPR and First Aid Certification within 30 days of hire and must be from one of the following certifying. organizations:
- American Red Cross
- American Heart Association
- American Safety & Health Institute
