Community Bridges is looking for a part-time 
Nursing Coordinator in Concord, NH!

We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.

 


The Nursing Coordinator will support the Nurse Trainers with administrative and quality assurance tasks, ensuring adherence to He-M 1201 Regulations. The position primarily supports Community Choices' Medical Staffed Residences, Behavioral Support Staffed Residences, Community Participation Services (CPS), and Enhanced Family Care (EFC) programs.

 

Qualifications:

Education: High School Diploma or GED required.

Experience: 2 years related experience as Licensed Nursing Assistant (LNA), Certified Nursing Assistant (CNA), Medical Assistant (MA), and/or Direct Support Professional (DSP), preferred

Reliable transportation, a valid driver's license, and proof of automobile insurance.

 


Compensation and Benefits

There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role.

  • Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Mileage Reimbursement: Employees are reimbursed for approved mileage driven on the clock.

Specific Duties and Responsibilities:

  • Maintain records for He-M 1201 medication authorizations, and assisting with scheduling for recertifications to be completed in a timely manner.
  • Maintain records for Nurse Trainers; to include records for all QA’s and other He-M 1201 appropriate documentation.
  • Assist with archiving of records.
  • Assist with preparation of semi-annual He-M 1201a reports. Transcribing data onto forms and tracking medication occurrences.
  • Assist nursing in preparation for annual state certifications to include transcribing data onto required forms.
  • Successfully complete Medication Administration Training.
  • As specifically identified and directed by the Nurse Trainer, meet additional support needs of staff to understand and master the He-M 1201 process and procedures.
  • Support providers to implement Healthcare Advocacy and to complete required healthcare paperwork correctly.
  • Conduct individual record reviews at EFC placements between nursing visits.
  • Assist nursing with development of new training and in large classes assisting with actual tasks in the class.
  • Assist with the gathering of health and medication information and setting up of health and medication books for the start of new programs.
  • Communication with medical provider offices and pharmacies.
  • Other duties as assigned.

 


Required Skills:

  • Familiarity with He-M 1201 medication administration guidelines.
  • Basic computer skills to include use of excel spread sheets.
  • Strong organizational skills.
  • Strong verbal and written communication skills.
  • Ability to collaborate with a dynamic team.

 


Work Environment and Physical Demands:

  • Work Environment – Must be able to maintain a positive and professional demeanor in busy program environment. Ability to navigate to program sites, including private homes that may include walk from parking spot, stairs, close spaces, etc.
  • Physical Demands – Must be able to occasionally carry program books, up to 25 pounds.

 


Travel:

Position will be split between 50% of time in the main office in Concord, 50% of time supporting programs on-site (within Merrimack County catchment area). Must possess and maintain a valid NH license and safe driver’s record in accordance with Community Choices standards with adequate liability insurance.
