What We’re Looking For:

Rock Creek Foundation is seeking a full-time Program Manager for Community Outreach to join our team in Silver Spring, MD! As the Program Manager of Community Outreach, you will be responsible for providing integrated goal oriented individual and group activities, while coordinating opportunities of interdepartmental synergy and a broad-based continuum of training and learning opportunities for the individuals we serve and all staff for consistency.

  • Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required.

What You’ll Do:

  • Helps train day program staff and maintain an environment of collaboration with other departments to better serve consumers within the program.
  • Assists program staff in planning and adapting materials and activities to enhance the consumer’s day, while ensuring safety and participation.
  • Collaborates with DDA program staff by meeting with consumers served once a week to ensure that consumers have scheduled community based activities and maintains written progress notes and consumer charts.
  • Performs regular audits of iCM to ensure that attendance and billing is accurate, timely, and justifiable for services rendered.
  • Conducts and manages LTSS/iCM training based on departmental needs and/or staff requirements.
  • Facilities departmental staff meetings.
  • Attends mandatory Company and department meetings as well as external meetings to stay abreast of changes in service, legislation, and to ensure programs are in compliance with State Waiver Requirements.
  • Completes screening intakes for program growth.
  • Attends annual person centered planning meetings for consumers served.
  • Monitors, corrects, and instructs consumer’s behavior, de-escalates anger and frustration, assists consumer in conflict resolution from harming self or others; keeps consumer attention focused on activities or work duties.

What We Require:

  • Bachelor's degree from four-year college or university OR a minimum of three (3) years related experience and/or training OR an equivalent combination of education and experience working with adults diagnosed with varying mental health and intellectual disabilities.
  • Must have access to a vehicle to use for business purposes and possess a valid Driver’s License (with no more than two points) and proof of current automobile insurance.
  • Must be able to work with “high-risk” consumers who have mental illnesses as well as developmental and physical disabilities.
