This is a unique opportunity to improve the lives of youth.

Sow A Seed Community Foundation is seeking a Youth Mental Health Specialist for its youth prevention and intervention program. The Youth Mental Health Specialist will work on school site campuses with vulnerable youths. The Youth Mental Health Specialist will: perform needs assessment analysis (CANSA); address a treatment or service plan; coordinate integrated student support through case management; provide life skills training and lead group workshops that focus on pro-social behaviors; assist youth and families with outside resources as needed.

Responsibilities
* Collaborate with school staff to assess, identify and prioritize student needs;
* Provide overall case management to identified students and deliver evidence-based interventions and supports, based on student and school needs;
* Regularly and systematically collect, enter and report student data;
* Connect with the parent or legal guardian of identified case-managed students;
* Provide timely reports with data and program information;
* Provide leadership, as requested, in securing community resources and partners beneficial in improving students’ physical and academic needs;
* Guide at-risk youth participants through a Behavior Intervention and Management Analysis;
* Assist with recommendations for current in-house programs;
* Complete referrals for further assessments, and behavioral or mental health services;
* Facilitate one-on-one and group sessions that actively promote healthy lifestyle choices as well as optimal emotional, social, and mental health.

Qualifications
* Minimum 2-years’ of applicable work experience working with adolescent populations;
* Experience working with youth development models;
* Bachelor’s Degree: preferred degree or enrollment in Psychology, Social Work, Therapy, or Counseling programs;
* Experience leading group facilitation;
* Good organizational and follow up skills;
* Ability to work well with minimum supervision;
* Strong interpersonal skills and ability to work with diverse community groups;
* Excellent verbal and written communication skills;
* Case Management experience a plus;
* Medi-Call Billing and or processing knowledge a plus;
* Spanish Speaking a plus;
* Must be able to pass a live-scan background check;
* Must also clear additional screening checks, which may include, but are not limited to:

  • - Professional licensing check;
  • - Exclusion Screening List;
  • - NPPES Data Verification;
  • - Educational credentials

* Must be committed!

Job Type: 20- 30 hrs. per week, during school hours (some positions maybe 10 months, September - May - summers off).

Salary: $21.00- $22.00 /hour

Job Locations: Tracy, CA

COVID-19 considerations:
Work with a small group of students in person and remotely during the distanced learning period. Must be available between the hours our 10 AM - 6 PM
