CLINICAL CARE COORDINATOR (Ambulatory Care - West Ann Arbor Health Center)

Job Description

Responsibilities*

  • Coordinates and oversees WAA Orthopaedic Surgery ACU patients who have been scheduled for surgery to support improved surgical outcomes, patient safety, efficient utilization of OR resources as well as improve faculty, staff and patient satisfaction.
  • Coordinates care pre-operatively for patients already scheduled for surgery for orthopaedic patient populations including, but not limited to pre-op clearances, lab result follow-up, medical record review for surgical risk factors (CAD, DM, DVT, PE, BM, CKD, stents), need for medical clearances
  • Manages peer to peer reviews and prior authorizations 
  • Prepares one-on-one consent for patients not attending education class
  • Develops and distributes weekly OR list for Ortho team
  • Communicates to patient cancelation of surgery and provides referrals to other providers/departments/procedures
  • Coordinates post-surgical follow-up and care in the immediate post-discharge period for orthopaedic patients including, but not limited to inpatient rounds to identify post-discharge needs, triage wound issues, take live calls via ASPECT to respond to post-operative surgical patients within their 6-week postoperative period, and complete post-discharge phone calls within 48-72 hours post-operatively to every surgical patient
  • Extensive knowledge about orthopaedic care and chronic care management is a critical skill for this position.
  • Provides tele-health nursing to patients and families to include: triaging calls from patients and families, clinicians, and other healthcare team members.
  • Collaborate with other health team members and associated resource persons to ensure continuity of care across all care environments. 
  • Assist in the development and implementation of competencies, protocols, and patient education tools to improve patient care.
  • Candidate must be able to function with a minimum of supervision and be highly motivated to provide excellent care and patient advocacy.
  • Participates in quality improvement initiatives.

Hourly range for RSAM Registered Nurses (12+ months experience) $37.24 - $57.91 / hour

? Evening Shift Differential- $2.55 / hour
? Night Shift Differential- $3.40 / hour
? Day Shift Weekend Differential- $1.75 / hour
? Evening Shift Weekend Differential- $4.30 / hour
? Night Shift Weekend Differential- $5.15 / hour
? Charge Nurse Differential- $1.00 / hour

Required Qualifications*

As evidence by interviews, recent performance appraisals, and reference feedback:

  • Current license to practice as a registered nurse in the State of Michigan 
  • Required Educational Requirement; applicant must meet one of the following:

Bachelor?s degree in Nursing OR

Associates degree or diploma in Nursing and a Master?s degree or higher in Nursing

  • Minimum 1 year RN experience (within the past 4 years) caring for adult orthopaedic patients.
  • Recent ambulatory care experience (within last two years) in an adult surgical setting.
  • Minimum 1-year RN experience in telephone triage of patient populations with complex and chronic conditions.

Desired Qualifications*

  • Previous Clinical Care Coordinator experience.
  • Demonstrated ability to provide leadership for clinical project/program.
  • Demonstrated participation in quality improvement initiative. 
  • Demonstrated excellence in communication skills; both verbal and written--role requires the ability to plan and develop patient education tools, protocols, and clinical competencies
  • Demonstrated excellence in critical thinking skills and problem solving skills in daily practice across the continuum of care for surgical patients and their families: assessment, planning, intervention and evaluation of care.
  • Demonstrated customer service and conflict management skills.
  • Demonstrated ability to establish collaborative practice with multidisciplinary team.
  • Demonstrated ability to direct nursing practice and delegate to clinical support staff
  • Demonstrated ability to function independently.
  • Demonstrated skills to organize and manage multiple interventions simultaneously understanding acuity and urgency in prioritizing duties.
  • Proficiency in computer and keyboarding skills, using an electronic medical record, and Microsoft Office and calendaring experience

Work Schedule

Hours: 40 hours per week
Days/ Shift:  Day shift; variable days; variable hours; 8-hour shifts
Location: West Ann Arbor, 

This position requires the nurse to be physically on-site

 

Note: All new employees will be expected to float to a designated unit(s) in times of low census.

This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
