Adult Day Health is a community health program that may be different than your last social service role. You will have the opportunity to run engaging and therapeutic group activities, with the support of a multidisciplinary team dedicated to working together to support our clients in meeting their health goals. You will have autonomy in your work. We want your expert care to better serve our clients. Unlike other health care settings our clients are under our care for years, and you will have the opportunity to form long-term professional relationships.

About Adult Day Health

Adult Day Health provides skilled nursing therapy, rehabilitative therapies along with transportation, meals, personal care, and social services to adults with chronic health conditions. All our programs are focused on supporting the independence of older adults and people with chronic illnesses and disabilities who wish to continue living in their community.

Adult Day Health is in person 5 days a week Monday-Friday. Staff are required to be in the office during program hours, and we plan to continue to offer flexible work arrangements to the extent that we are able.

Full Life Care is a 501-c3 not-for-profit organization, founded in the 1970s that provides adult day health, home care, housing stabilization services, and care coordination in the greater Seattle area. Full Life Care is dedicated to enhancing the quality of life for frail elders and people with chronic or terminal illnesses and disabilities.

We are committed, through our program of health and social services, to the independence and well-being of all participants and to providing respite for caregivers. Full Life Care is working toward a future where adults with serious illnesses or disabilities can be supported in the community rather than living in isolation or needing to resort to institutional care.

ESSENTIAL FUNCTIONS of the job:

  • Determine clients’ situation and needs by establishing rapport with clients; collaborating with client, caregiver and team members to establish service goals and plans;
  • Addresses client personal care needs by assisting clients in restroom, with ambulation, meals and hygiene as needed;
  • Maintains continuity of care by communicating changes in client condition to team members;
  • Maintains a safe and healthy environment by adhering to infection control protocols; addressing potential safety and health hazards; following emergency policy and procedures; and fulfilling education and certification requirements;
  • Promotes client independence, education and recreation by developing and facilitating activities according to clients’ abilities and interests; encouraging interaction and participation;
  • Develops a safe, therapeutic group environment by establishing and keeping group rules; monitoring and encouraging appropriate participation; intervening as necessary;
  • Establishes a compassionate and respectful environment by practicing good communication skills; keeping client information confidential; conducting self in professional manner;
  • Light administrative work; coordinating transportation;

MARGINAL FUNCTIONS of the job:

  • Maintains personal care supplies inventory by checking stock level; anticipating needs; informing appropriate team member when supplies need to be ordered;
  • Complete projects and tasks as assigned by supervisor;

Educational Requirements Needed to Perform the Duties of the Job:

  • Must have a high school diploma or equivalent;
  • One or more years of experience working in a health or social services setting preferred;
  • Must complete the HIV/AIDS information course;
  • Employees are required to receive an annual influenza vaccine unless granted a medical or religious exemption.
  • Employees are required to be fully vaccinated against COVID-19 unless granted a medical or religious exemption.

Physical Requirements Needed to Perform the Duties of the Job:

  • Must be able to lift 25 lbs. and support clients’ mobility.
