Summary

 

To provide comprehensive professional nursing support in a multidisciplinary pediatric dialysis clinic, supporting the hemodialysis and peritoneal dialysis populations.  Coordinate care and follow-up for patients with chronic kidney disease across the care continuum.  Provide expert nurse instruction and consultation to members of the health care team.  Identify gaps and needs of the patient population and seek to implement interventions to improve patient outcomes.  Develop partnerships with other clinical programs to standardize patient care processes.  Facilitate and maintain quality improvement / LEAN activities.

Responsibilities*

• Provide expert nursing care to the Hemodialysis and Peritoneal Dialysis patient population through individual assessment, evaluation and monitoring of treatment.

• Coordinate and assess the nursing care given to the Hemodialysis and Peritoneal Dialysis patient population in multiple hospital or clinical units

• Design, monitor and evaluate nursing care programs and individualized patient care plans.

• Provide liaison and consultation in the area of clinical expertise to members of the healthcare team in multiple units, services and disciplines.

• Document care and patient progress through detailed patient records.

• Initiate and participate in the design and maintenance of clinical area quality assurance programs and activities including SCOPE.

• Supports standardization and development of departmental protocols, policies, and procedures using evidence based practice.

• Demonstrate knowledge of current medical, surgical and pharmaceutical treatment modalities. Pharmaceutical adherence and monitoring

.• Provide education to the Dialysis patients, families and community.

• Design, develop and maintain patient / family education programs.

• Assist in the development/revision of patient educational guidelines and materials for hemodialysis and peritoneal dialysis.

• Conduct in home visits as needed.

• Participate in ongoing research studies in compliance with federal and organizational regulatory guidelines.

  • Evening Shift Differential- $2.55 / hour

  • Evening Shift Weekend Differential- $4.30 / hour
  • Night Shift Weekend Differential- $5.15 / hour
  • Charge Nurse Differential- $1.00 / hour

Required Qualifications*

Current licensure to practice nursing in the State of Michigan

Bachelor’s degree in Nursing OR

Associates degree or diploma in Nursing and a Master’s degree or higher in Nursing

 Dialysis experience-  2.5- 3 years of current pediatric dialysis experience.

  • Demonstrated excellent critical thinking and problem solving skills.
  • Inpatient pediatric experience.
  • Demonstrated ability to work collaboratively with and positively contribute to a team approach to patient care, willingness to flex and cover colleagues.
  • Previous experience with quality improvement / LEAN projects.
  • Demonstrated high standards of personal and professional conduct and attendance as evidenced by interviews, references, and performance evaluations. 
  • Demonstrated high level of self-motivation to learn and stay current in the treatment of chronic kidney disease. 
  • Must demonstrate high level of self-direction and flexibility.
  • Must demonstrate expert communication, organization and prioritization skills.
  • Previous experience in the development of evidence based clinical protocols.
  • Previous experience in professional presentations and group education.
  • Demonstrated knowledge and experience in planning, developing, and implementing complex Continuous Quality Improvement activities.
  • Previous participation in clinical research.
  • Experience in program development, research and regulatory readiness program planning.

Work Schedule

Hours/ Shift : 32 hour Day shift, Monday- Friday. 

Location: Michigan Medicine Pediatric Dialysis Unit

This position is covered under the collective bargaining agreement between the U-M and the Michigan Nurses Association and the U-M Professional Nurse Council union, which contains and settles all matters with respect to wages, benefits, hours and other terms and conditions of employment.
