CLINICAL CARE COORDINATOR (Congenital Heart Center- Comprehensive Outpatient Preoperative Assessment)

Summary

The Pediatric Cardiology COPA CCC provides comprehensive coordination of care to Congenital Heart Pre- Operative and Pre- Procedural patients and works collaboratively with colleagues in the Mott PACU, Mott OR and Cardiology clinics. The clinica office RN will coordinate pre-operative/ pre- procedural preparation and provide nursing care to patients with complex cardiac conditions. It is required that the RN collaborates with other health care professionals: Physicians, referring offices, schedulers, Cath lab nursing, diagnostic services, and pharmacists to provide a coordinated plan of care for our patients.  He or she will review patient medical records from referring physicians and establish diagnosis and treatment plan with the medical staff.  The coordinator may identify the need to order lab-work and other testing.

The Pediatric Cardiology COPA CCC screens all Congenital Heart Pre-Operative patients being seen in clinic, to identify problems that may lead to complications.  Telephone management and excellent communication skills are essential to this position.  The position includes a thorough evaluation of medical history for out-of-state or long distant patient population using communication mechanisms including fax, telephone, and electronic mail.

Evaluation of the daily scheduling of patients in coordination with each patient's individual testing needs requires the expertise on the nurse's behalf. The RN must incorporate knowledge of each specific cardiac procedure as well as the individual patients testing and preparation requirements to ensure the patient-care flow through the department and UMHS during the day of procedure is as efficient and safe as possible.

  • The CCC works collaboratively with colleagues to develop, evaluate, and revise patient education programs and materials. Additionally, the CCC participates in data collection for COPA and other QA/QI endeavors related to MCHC population.

Responsibilities*

  • Provide coordinated care to pediatric patients and families in the Pediatric Cardiology COPA clinic.
  • Participate in all aspects of care coordination as a member of a collaborative and multi-disciplinary team.
  •  Provide patient-and-family-centered-care in the outpatient setting and tele-health management.
  • Tele-health management includes triaging calls and in-basket messages from patients, families, clinicians, and other healthcare team members.
  • Communicate with referring clinicians and other healthcare team members.
  • Work in an integrated fashion with physicians, physician assistants, nurse practitioners, nutritionists, discharge coordinators, and other consulting services to provide high quality patient care for outpatients
  • Provide professional nursing support to Cardiac Surgery and Pediatric Cardiology including, first contact from cardiology service, and coordinate preoperative diagnostic studies.
  • Pre-operative teaching and counseling for MCHC pediatric surgical procedures via phone screening and in person clinic visits.
  •  Maintain patient flow in clinic, and support assistive staff as needed.
  • Collaborate with clinicians, other health team members, and associated resource persons to provide continuity of care.
  • Assist in the development and implementation of standards and protocols in collaboration with the clinical management team.

  • Develop and update electronic documentation of preoperative patient telephone call
  • Participate in meetings involving Surgical Site Infection prevention, North Star pre-op team, COPA leadership team, and surgical conference.
  • Partner with Admission and Bed manager for pre-admissions
  • Maintain a liaison relationship with leads in Nursing, and other ancillary services, Blood Bank, Patient Relations, Registration, Cardiac Surgery Team, PACU, Genetics, and Dental services.
  • Participate in performance improvement activities
  • Partnership with outreach referring cardiologists
  • Provide orientation to new nurses in COPA clinic
  • Participates in data collection for COPA and other QA,/QI endeavors related to MCHC population.

CCC- COMPETENT.

Actual RSAM LEVEL and salary will be determined at time of hire.

RSAM levels range from CCC-COMPETENT to CCC-EXPERT TO CCC-MASTERY to CCC- MASTERY PLUS

Required Qualifications*

  • Required Educational Requirement; applicant must meet one of the following:

          Bachelor’s degree in Nursing OR

          Associates degree or diploma in Nursing and a Master’s degree or higher in Nursing

  • Must hold current license to practice nursing in the State of Michigan.
  • Minimum 3 years in Pediatric Congenital Heart Disease nursing experience
  • Outpatient phone triage for patients with Congenital Heart Disease

Desired Qualifications*

  • Demonstrated understanding of hospital organization and processes.
  • Demonstrated knowledge and experience in planning, developing, and implementing complex Continuous Quality Improvement activities.
  •  Demonstrated active participation in formal unit improvement committees including but not limited to Professional Practice Councils, or other committee or QI work.
  • Demonstrated expert clinical leadership abilities in assuring patients are properly prepared for procedures.
  • Must be detail oriented, organized, and be able to manage rapidly changing outpatient scheduling issues.
  • Demonstrated ability to anticipate the clinical care needs of complex patients.
  • Highly motivated and self-directed with demonstrated ability to function independently and as a team member of the Congenital Heart Center team with functional leadership responsibilities for the scheduling/scheduler position.
  • Must demonstrate a high level of independent problem solving and decision-making abilities.
  • Demonstrated expert analytical skills and ability to prioritize assignments and manage busy work environment.
  • Must reflect a professional appearance and demeanor and possess excellent customer service skills.
  • PALS certified.
