OVERVIEW

Under the leadership of the Field Office Director, the Health and Wellness Program Manager is responsible for managing a portfolio of programs within USCRI Des Moines, as well as the direct supervision of program staff. The Program Manager will oversee State and Federally funded programs as well as private foundations to help clients achieve their goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Ensure Team staff are productive, meeting all contract requirements, and nurturing a positive team environment;
  • Understands State, Federal, and Local program contracts in detail, ensuring all contract requirements are met and opportunities maximized;
  • Provide ongoing leadership, direction, development, coaching, counseling, support, and oversight to program staff;
  • Direct and coordinate implementation of programs within the position’s portfolio;
  • Responsible for meeting all programs reporting deadlines, ensuring reports and vouchers are complete and correct according to contract requirements and guidelines;
  • Foster strong working relationships with hospitals, medical clinics, community-based organizations, and partner agencies; provides consistent communication with partner agencies to ensure coordination of services and development of client and funding opportunities;
  • Provide health care-based case management, including intake, assessment, medical appointment supports, advocacy, and other services to clients in order to help achieve self-sufficiency goals;
  • Attend office and program staff meetings; and
  • Perform other duties as assigned by supervisor.

POSITION REQUIREMENTS

  • Bachelor’s degree from an accredited institution, preferably in Human Services, Social Work or related discipline;
  • Excellent interpersonal skills, including the ability to communicate well, building relationships and partnership with USCRI clients, community stakeholders, state and federal program staff, headquarters, and field office staff;
  • Proficient in the use of MS Word, MS Excel, Outlook, and database programs, as needed;
  • Ability to get along with others and thrive in a dynamic fast- paced team environment;
  • Demonstrated commitment to the mission of USCRI and interest in contributing to the long-term success of a local nonprofit organization;
  • Must have a clean valid driver’s license and insurance, and a reliable motor vehicle; and
  • Demonstrated support of USCRI’s values by exhibiting respect for others, maintenance of confidential information and an appreciation of a multicultural workplace.

TRAINING REQUIREMENT

  • Satisfactory completion of USCRI’s Orientation and Training; and
  • Complete additional training as identified by supervisor or Human Resources.

PHYSICAL DEMANDS

  • Use of manual dexterity, tactile, visual, and audio acuity;
  • Use of repetitive motion, prolonged periods of sitting and standing, and sustained visual and mental applications and demands;
  • Occasional lifting (up to 25 pounds), bending, pulling, and carrying; and
  • Quantitative/mathematical ability (addition, subtraction, multiplication, division, standard measurements.
