Pay Status: Salary-Exempt
Work Schedule: Monday through Friday – flexibility to work from home

Harbor Care, formerly known as Harbor Homes and the Partnership for Successful Living affiliates, is an innovative health and human services non-profit organization that provides housing, healthcare, home health, behavioral health, addiction treatment, and more in collaboration with many community partners. Harbor Care is the new shared name of Harbor Homes, Keystone Hall, Healthy at Home, the Harbor Care Health and Wellness Center, and the Southern NH HIV/AIDS Task Force. Job Summary:

  • This position is a mix role of case management, outreach and support for the systemic data needs of the Supportive Services for Veteran Families program.
  • Responsible for coordinating with DHHS in the refinement and maintenance of the NH Veterans Master List.
  • Facilitates data sharing discussions with local data systems to effectively capture all veteran households experiencing homelessness, and develop a working system response around these measures.
  • Represent the SSVF program at committees around the state, reporting out on data, identifying trends and gaps in the COC.
  • Coordinate housing retention rates and data measurements post SSVF discharge.
  • Operates in the context of true community collaboration involving all stakeholders who provide services to Veterans and their households; Continuums of Care, VA…etc.

Basic Qualifications:

  • Bachelor's degree, in social work or behavioral science, Master preferred
  • One to two years of previous experience in adult case management.
  • Working knowledge of community resources as they relate to homelessness and veterans.
  • History working with vulnerable/homeless populations
  • Experienced with Microsoft Office (Word, Excel, PowerPoint), Outlook, Internet
  • Veterans preferred

Essential Job Functions:

  • Collaborates with stakeholders in the community to ensure that all Veterans experiencing homelessness within NH are identified, their housing needs are known and refers to SSVF as well as other housing services as appropriate.
  • Develop relations with town/city/county/state/federal/veteran resources to locate participants.
  • Measures progress with veteran subcommittee goals towards achieving, and then maintaining an Effective End to Veteran Homelessness in all 3 COCs.
  • Identify key barriers to goal attainment and opportunities to resolve them
  • Will help develop workflows for each data source regarding the Master List, case conferencing and prioritization across the state.
  • Will facilitate data sharing practices between local systems including Homeless Management Information Systems (HMIS), VA’s HOMES, …etc to effectively capture all veteran household data in the pursuit of Ending/Maintaining an Effective End to Veteran Homelessness.
  • Will represent SSVF in all data and RRH subcommittees, coordinate presentations and report out on trends and gaps for use in the COC.
  • Will work with COCs Coordinated Entry committees on system data, quality assurance, policies and procedures to ensure compliance with HUD and VA priorities.
  • Will measure and report on housing retention rates for household’s post- discharge from the program, identifying trends and areas for improvement.
  • Network and outreach with other agencies, coalitions, and local community meetings
  • Actively participate in staff meetings and complete trainings as assigned
  • Other duties as assigned by Program Manager

Knowledge, Skill and Technical Qualities:

  • History working with Veterans and with vulnerable populations
  • Veteran status preferred, but not required
  • Experience/knowledge of computers applications
  • Be aware of and adhere to all HIPAA rules and regulations and must be an active participant in safety measures for ensuring confidentiality of information as it relates to clients and this agency
  • Ability to work independently
  • Strength based perspective, with a drive for social justice
  • Ability to work effectively with people, and function as part of a team
  • Respect of client diversity and/or families cultural, religious, and ethnic differences
