Williamsburg Landing, a 5-star CARF-accredited life plan community, seeks a Memory Support Manager to oversee the day-to-day operations of nursing care to residents in Memory Support, including their physical and psychosocial well-being. The Manager serves as the practicing LPN and oversees the work assigned to other LPNs, RMAs and CNAs and directs emergency response when necessary. In addition to working closely with Dementia Services and interdepartmental staff to create a cohesive environment to care for the psychosocial needs of the residents living with dementia, the Memory Support Manager assists physicians as requested, ensures all documentation is timely and correct, all medications are dispensed as written, and that all errors are promptly reported and remediated.
ESSENTIAL JOB FUNCTIONS

  • Assists in general management and leadership of Memory Support; responsible for oversight and operational management performance
  • Oversees the work of nursing staff, ensuring that responsibilities are performed according to expectations; assigns work and trains staff in accepted work methods; participates in hiring and evaluating staff; disciplines staff as needed and makes recommendations for termination when required
  • Monitors staff for professionalism and positive interaction with residents and their families, guests, and other Williamsburg Landing employees; coordinates with Dementia Services to support a culture of relationship-centered care through parallel programming, and personalized one to one engagements
  • Coordinates with the Manger of Assisted Living to develop staff schedules; updates the stay-over list and assists with call outs/replacements coverage
  • Performs RMA and LPN observation audits regarding the passing of medications; reviews/trains staff, as needed, on policies and procedures regarding medication management, and completes employee performance evaluations and Stay Interviews
  • Ensures all shift responsibilities are completed before the end of the shift by the LPNs, RMAs and CNAs or that the next shift's expectations have been communicated during shift change report
  • Makes rounds daily on staff and residents for effective communications
  • Responds to resident needs including emergencies, medication administration, and support with activities of daily living when needed
  • Ensures that schedules have been created, posted and communicated, and coordinates follows up on all medical appointments and consultations for residents; prepares paperwork for outside appointments; may request assistance from nursing staff or volunteers if appropriately trained
  • Maintains vigilance to observe any unusual symptoms or signs of illness or skin breakdown, or negative changes in vital signs; reports any indication of residents' deteriorating physical, emotional, or mental health by noting such signs or evidence as uneaten food, deteriorating cleanliness, or conversations that indicate deep unhappiness or loneliness
  • Contacts the physician as needed; assists the physicians with medical procedures when requested
  • Ensures that resident prescriptions are ordered if needed; disposes of medications according to established protocol when needed
  • Ensures resident weight sheets and vital signs are completed/updated each month as required by regulation or policy, coordinating follow up by Care Team as indicated; may request assistance from nursing staff
  • Ensures all updates of Activities of Daily Living Task Sheets are completed
  • Conducts chart audits for Memory Support in preparation for the annual Department of Social Services (DSS) Survey; assures residents are charted on a monthly and as-needed basis and completes all required documentation within the prescribed guidelines
  • Completes Uniform Assessment Instrument (UAI) and Individualized Service Plan (ISP) training for developing and implementing resident care plans; assists with resident evaluations to determine individualized care plans; attends and actively participates in resident review team meetings as needed
  • Responsible to ensure daily thermometer checks on the refrigerator holding medications and monthly checks on the emergency bag and emergency carts; may request assistance from nursing staff
  • Checks for expired and discontinued medications
  • Maintains communication with family member as requested and within the confines of federal and state regulations; participates fully on teams established to improve processes and communication between the unit and families or other segments of Williamsburg Landing
  • Maintains adequate supplies and equipment for the unit and records supplies and equipment to be ordered
  • Maintains positive and professional relationships with residents, guests, and other Williamsburg Landing employees
  • Evaluates work processes and methods and submits recommendations for continuous performance improvement
  • Ensures that staff receives training to meet mandated standards, that develops their professional abilities, and that prepares them for advancement opportunities
  • Assumes responsibility for own professional development; attends scheduled training programs that includes, at a minimum, trainings required by Williamsburg Landing and by regulatory and accrediting bodies, as well as staying abreast of developments within the field
  • Performs other duties as assigned

QUALIFICATIONSMust possess a current license as a Licensed Practical Nurse in the Commonwealth of Virginia; ability to read and write effectively in English; two (2) years of nursing experience in long-term care or assisted living setting with at least one (1) year of experience working with people with advancing stages of dementia, supervisory experience is preferred
KNOWLEDGE, SKILLS, AND ABILITIESAbility to communicate effectively in English both verbally and in writing to fulfill job responsibilities; comprehensive knowledge of nursing procedures, theory and practice with an emphasis on current trends in gerontological nursing; knowledge of different types of dementia and how they present; knowledge of programs that delay the progressive cognitive deterioration of people with dementia; ability to work effectively as a member of a team and to support the development of LPNs, CMAs, and CNAs; exhibits effective leadership skills in managing staff, proficient with MS Office and internet-based research; ability to exercise sound professional judgment; ability to respond appropriately in an emergency; ability to lift and carry a minimum of 50 pounds; requires standing and walking for entire shift, lifting and transferring residents, and maintain current CPR
Employees may be required to work additional hours when business needs make such scheduling necessary.
