Full Job Description

The program coordinator will serve as the practice facilitator for the Screen Up Project. He/She will work with primary care practices for successful implementation of evidence based interventions, population health strategies, and quality improvement activities to establish organized approaches to cancer screenings within clinical settings. This position is responsible for managing and cultivating relationships, assessing clinical workflow and processes, using practice facilitation, and coaching to drive quality improvements related to increase cancer screenings

Must have a Bachelor's in Public Health, Policy, Administration, Health Communications, Quality Improvement or a related field and three years experience in health care quality improvement. A Master’s Degree with 1 year of professional level experience in health care quality improvement may substitute for required experience.
