The Director of Case Management Care Coordination will work with a non-profit reputable healthcare system with medical centers spread across six campuses and a medical foundation of physicians at care centers delivering approximately 10,000 babies, treating 60,000 patients, seeing 185,000 people in our emergency departments, and providing more than $120 million in community benefit and charity care to the communities.
The Director of Care Management Care Coordination will be part of a growing, expanding and financially stable organization that offers comprehensive health care services with a network of primary care physicians, specialists and hospitals offering specialty services employing more than 8,500 employees and 2,100 physicians.
With average daily census of 700, the Director of Care Coordination Case Management will work closely with the interdisciplinary and Administrative Team to align Case Management operations with facility goals and targets as well as Regional Care Coordination and System Affordability goals. This position works with the Hospitalist Program Medical Director to align operations to achieve targeted outcomes.
The Care Coordination and Utilization Review Director also collaborates with the facility Chief of Staff, UR Physician Advisor, VPMA, and Utilization Review chair to carry out the facility utilization management program annually. The Senior Nurse Director Director of Case Management is responsible to work with Nursing and Department Directors to ensure maximum reimbursement for care within contractual and regulatory guidelines.
